When you submit the prequalification form, we collect basic contact and routing information: your name, email address, state of residence, the goal pathway you're interested in, and your consent to be contacted. We do not collect medical history, diagnoses, medications, allergies, lab results, photos, or other Protected Health Information through the public website.
Once you are routed into a clinical intake operated by a licensed clinician through a partner platform, that platform collects medical information separately and governs it under its own HIPAA-compliant Notice of Privacy Practices. YourHealth does not receive, store, or process Protected Health Information (PHI) as part of that clinical workflow.
2. How we use information
- To confirm whether YourHealth services are available in your state.
- To follow up with next steps and route eligible individuals to a licensed clinician.
- To improve the website and the prequalification experience.
- To measure marketing and content performance using non-identifiable analytics.
- To meet legal, regulatory, and operational obligations.
3. Sharing and contracted partners
We share information only with vetted service providers who help us operate the website (for example, hosting and email infrastructure) and with contracted clinical platform and provider partners who deliver care if you proceed past prequalification.
Our primary contracted clinical platform partner is Ola Digital Health. If you proceed through intake, your prequalification routing information may be shared with Ola Digital Health and its affiliated licensed clinicians and pharmacy partners for the purpose of delivering telehealth services. Ola Digital Health operates as a contracted service provider and handles clinical data and PHI under its own HIPAA-compliant practices and separate Notice of Privacy Practices.
We do not sell personal information to third parties for advertising, profiling, or any commercial purpose unrelated to delivering the services you requested.
4. HIPAA and clinical data
YourHealth is not a Covered Entity under HIPAA with respect to public website interactions. When you are routed to a clinical intake, the licensed clinician and the clinical platform partner (Ola Digital Health) act as the HIPAA-covered parties responsible for the privacy and security of your Protected Health Information. That relationship is governed by the partner's Notice of Privacy Practices, which you will be presented with before any clinical intake begins.

5. Data security
We use reasonable technical and organizational safeguards to protect information you submit through the public website. No method of transmission over the internet is 100% secure; please do not submit sensitive medical details through the public prequalification form.
6. Your choices
You may request access to, correction of, or deletion of your personal information by contacting our support team. Some information may be retained where required by law or for legitimate business purposes. Requests related to clinical records should be directed to the partner clinical platform.
7. Analytics and tracking
We use privacy-preserving analytics tools on the public website to understand traffic patterns, content performance, and conversion behavior. These tools do not collect PHI and do not use third-party advertising cookies. Analytics data is collected at the aggregate or anonymized level only.
8. Children
YourHealth services are intended for adults 18 and older. We do not knowingly collect information from children.
